Recognize your Coworkers

Take a moment to nominate someone today for FOD’s Annual Recognition Awards to be presented at WinterFest. The two annual awards are (mark one of these two boxes on the nomination form):

• Special Recognition - For individuals and teams that may be a part of initiatives that are not visible to all employees throughout the year, including partnering with other areas within Ohio State or the surrounding community, overall leadership, or other significant achievements.
• Innovation Award - Recipients are recognized for developing and/or participating in a project or process improvement that moves FOD toward its mission and substantially impacts operations. Examples include improving the quality of delivered services or the modification of practices that can result in cost savings for the university.

Want to save $240?

Complete your PHA by November 30 - Employees are encouraged to complete their Personal Health Assessment by November 30 to maximize their 2010 medical plan savings. On January 1, the medical plan premium reduction for completing a PHA will increase to $240. This amount is prorated monthly, so it is important to complete the PHA by November 30 to realize the full $240 savings. Schedule a biometric health screening to obtain your health numbers for the PHA at YourPlanforHealth.com.

Paper Option - If you do not have ready access to a computer, you can complete the PHA on paper. Pick up the forms from Kim Fiebig (FOD HR, 143 McCracken, fiebig.6@osu.edu, 688-5802).

 

More Purchase Request Form & Procurement Training Sessions

If you use a FOD Purchase Request and would like more information on filling it out correctly, please come to a Purchase Request and Procurement Training. This is open to all FOD employees and is especially helpful to those who use these forms on a daily basis. It will help you understand which boxes need to be filled in and what information is needed to help get your PO or Requisition in a timely manner. Please sign up for one of the following sessions by contacting Sandy Otis (otis.12@osu.edu, 247-7928). All sessions will be at 1100 Kinnear Road, Room 200.

• Wednesday, November 18, 9 a.m. to 10 a.m.
• Wednesday, December 16, 3:30 p.m. to 4:30 p.m.

• Wednesday, February 17, 2 p.m. to 3 p.m.

WinterFest … in appreciation of The Work You Do

All FOD staff and their guests are cordially invited to celebrate at this year’s recognition event … Thursday, December 10, 1 p.m. to 3:30 p.m. (registration starts at 12:30 p.m.). You will receive an invitation mid-November. Please let us know if you will be attending and/or bringing guests. The event is free for all FOD staff, and guest lunches can be purchased for $5 at the door. The location is RPAC lower gymnasium. Please enter through the northwest corner doors.
