Report the accident to your supervisor and fill out an Employee Accident Report. When it has all of the authorizing signatures, submit it to Human Resources.
Temporary Handicapped Parking Pass
To obtain a handicapped parking pass, you must obtain a doctor’s prescription and submit it to a State License Agency for a small fee. They will issue an Ohio temporary hangtag and a certification letter.
Take the Ohio hangtag and certification letter to Transportation & Parking, and for no charge a university hangtag will be issued. You must display the university hangtag when parking on campus.
